What we do
Patients with a confirmed diagnosis of Parkinson’s, Multiple System Atrophy, or Progressive Supra Nuclear Palsy.
Supports patients and families with neurological conditions.
To prevent or reduce hospital admissions and to reduce consultant and GP visits.
Access to the service is at any point following diagnosis.
Access via referral form, completed by any health professional, unless in hospital then telephone referrals will be accepted.
