We provide high quality healthcare services primarily to the residents of East Lancashire and Blackburn with Darwen, which have a combined population in the region of 530,000.
As the result of a year- long service redesign programme, health experts across East Lancashire and Blackburn with Darwen have come together to improve the way diabetes care is delivered.
The service redesign has involved listening to what patients want and working closely with Consultants, GP’s, Diabetic Specialist
Nurses and other key professional staff.
The aims of the integrated service will be to ensure that you have the right level of care and support to meet your needs, at the right time and in the right place.
It is anticipated that the Enhanced GP practice and or in conjunction with the Community Specialist Service will provide the majority of care for patients with diabetes (up to 95%) with only 5% of patients requiring complex care at consultant level.
Whenever possible, the service will provide care closer to home either within a specialist community clinic or at the GP practice.
Enhanced GP practices will be supported to deliver diabetes care by other members of the integrated diabetes team, who will provide training and on-going mentorship.
