What does the service provide?

Our award winning team have supported care homes to achieve the ‘Gold Standard Framework for Care Homes’ including one beacon status and one highly commended.

Erin Bolton, Macmillan End of Life Lead was awarded the Innovation Award at the Macmillan Inaugural Professional Excellence Awards in 2012.

The Macmillan Specialist Palliative Care Team provide an holistic service to those with complex end of life conditions, mostly but not exclusively for those in the last twelve months of life.  Care support and advice is offered for symptom management, psychological, spiritual and emotional distress for patients, their families and carers.  

The team provides symptom management advice to generalist palliative care staff.

The End of Life Team aims to promote a high quality of end of life care for all.  The team work closely with primary and secondary health care professionals to adopt nationally recognised best practice tools: Gold Standard Framework, Preferred Priorities of Care, Liverpool Care Pathway.

In addition to this the team seek to enhance end of life care within the care home setting thus enabling individuals to remain within their own home when this has been stated as their preference.

What to expect?

  • Holistic assessment of the individual with a life limiting, progressive condition including physical, psychological, emotional, spiritual, social and financial factors.
  • Tailored individualised care plans to meet the assessed needs e.g. symptom management; taking into account the individuals’ / carers’ expressed wishes and preferences.
  • Referral to other supportive services where appropriate e.g. palliative day therapy, complimentary therapy, specialist palliative care in-patient beds where indicated.
  •  Provide formal education sessions for health and social care professionals across both primary and secondary care.
  • Consultancy based support and advice to generalist palliative care health and social care professionals.

The Macmillan Clinical Nurse Specialist team accepts referrals from health and social care professionals (with patient consent).   The patients GP / responsible physician are also made aware of the referral.  Patients their families and carers may also self refer to the service.

Places where services are delivered:

 The service is available across primary and secondary care regardless of setting.

 The service operates Monday – Friday 9.00 am – 5.00 pm excluding Bank Holidays.
